Liquor License Renewal Applicant
2025 Deadlines
- Applications will now be submitted thru the City of River Falls Licensing Application Portal
- Online application must be completed by May 8, 2025. A copy of your current Wisconsin Seller’s Permit must accompany your application.
- Fees are due by 5 p.m. Wednesday, June 4, 2025. All fees must be paid before licenses can be issued to the business.
- The license period is July 1, 2025, through June 30, 2026.
- If you would like to meet in person to submit your paperwork, please schedule an appointment by calling 715-426-3448.
Required Forms
The following forms will need to be completed and uploaded under the state forms section at the bottom of the online application.
- Renewal Alcohol Beverage License Application (AB-200 - All Sections need to be completed including current Wisconsin Sellers Permit Number and FEIN Number
- Alcohol Beverage Individual Questionnaire (AB-100)
- Alcohol Beverage Appointment of Agent (AB-101)
- Addendum to Renewal Alcohol Beverage License Application (Complete if your business has an outdoor area you would like to license)
- WI Sellers Permit - Include a copy of your current permit Liquor License Checklist
- Liquor License Renewal Checklist
- Supplement to Schedule For Appointment of Agent
A Change In the Business May Require One or More of the Following:
- Auxiliary Questionnaire and Authorization For Release of Information- Complete each form if there has been a change in the membership of the company. Each new member must complete a Questionnaire and an Authorization.
- Schedule For Appointment of Agent, Authorization Authorization For Release of Information and Supplement to Schedule For Appointment of Agent - Complete the Schedule and the Authorization if there has been a change in the agent for the business.
- Alcohol License Application Supplement - Complete if there has been a change in your business plan or the premise that needs to be licensed.
